経結膜的下眼瞼形成術に伴う眼瞼下垂様症状の改善

東洋人の多くに過剰な下眼窩脂肪の存在が報告されており、加齢に伴うこの脂肪支持組織の弛緩により、下眼窩脂肪が前方突出する。この症状がいわゆる baggy eyelidと呼ばれる状態で、目の下のたるみといった老化兆候として認識されやすくなる。また若年層で、下眼窩脂肪前方突出が顕著でなくとも、下眼瞼構造の不具合で、下眼瞼皮膚の下垂位により下眼瞼色素が強調された、いわゆる目の下のくま（クマ）症状を発症することが少なくない。これらの多くの目の下のくま（クマ）、たるみ症例を注意深く観察すると、上眼瞼の凹みや軽度眼瞼下垂症状を併発している場合が多い。目の下のくま（クマ）、たるみ解消を目的とした下眼瞼形成術を適切に行うと、これらの症状の改善ならず、上眼瞼の凹みや眼瞼下垂症が改善する例が多かった。

Ptosis Improvement after the Transconjunctival Lower Bleparoplasty

Japanese population has been known to inherit multiple kind of genes from various part of the world. Most Japanese poses the gene from mongoloid population more or less, which has a role to comprise relatively a large amount of lower orbital fat. This fat was explained to protect eye balls from cold climate in ancient time. On the other hand, Japanese who has a gene from southern asian population who use to inhabit in walm climate has less amount of lower orbital fat and small orbital structure containing it.
In case someone inherits an abundant amount of lower orbital fat from mongolian origin and small orbit capacity from southern asian population, this mismatch will bring about baggy eyelid because this excessive fat has no space to be placed. And also, tear trough appearance might be emphasized due to ptotic lower eyelid skin caused by unnecessary anatomical structure underneath it, which is created by genetic mismatch as explained above.
Among numerous cases of lower eyelid treatment, it was found that certain number of patients developed blepharoptosis tendency or narrowing of eye fissure. However, those symptoms seem to be improved after the transconjunctival blepharoplasty. Specific cases of blepharoptosis improvement with the transconjunctival blepharoplasty will be shown and also the mechanism of this improvement will be explained.

はじめに
眼瞼下垂症は老人性、ハードコンタクトレンズを長期装用した際に生じる筋膜性が原因であることがほとんどだが、そういった明らかな原因がなくても発症する眼瞼下垂傾向を認めることがある。さらに眼瞼下垂症には腱膜性ではなく、上眼窩脂肪や余剰皮膚が瞳を覆う偽眼瞼下垂症もあり、原因別に明らかに分類することは予想以上に困難である。
当クリニックでは、いわゆる目の下のくま（クマ）、たるみ治療に来院した患者の中に眼瞼下垂症を併発している例が少なくなかった。こういった症例では経結膜的下眼瞼形成術を行うことで、眼瞼下垂症状の改善を認めた。この下眼瞼形成術によって、何故眼瞼下垂症状が軽快するのか、その推測も含めて具体例を示す。
